Media Consent and Release

By ticking this box, you, as the participant or the parent/legal guardian of the participant named in this registration form, consent to Tetiana Tymofyeyeva (Wilczynska) trading as Therapeutic Art for Positive Wellness collecting, creating, and using photographs, videos, audio recordings, artwork images, testimonials, and other media that may include you or the participant during participation in any services, programs, classes, workshops, events, therapeutic art sessions, one-on-one (1:1) sessions, community programs, online sessions, educational activities, or other activities conducted by Therapeutic Art for Positive Wellness.

You consent to the use of such media for educational, promotional, reporting, marketing, fundraising, and business purposes, including publication in printed materials, newsletters, presentations, websites, social media platforms, online advertising, and other digital or multimedia communications operated by or on behalf of Therapeutic Art for Positive Wellness.

Important Information

By providing consent, you acknowledge and agree that:

• Media may be published, distributed, displayed, or shared in Australia and internationally.

• Information and images published online may be accessible to the public, indexed by search engines, copied, downloaded, shared, or otherwise used by third parties beyond the control of Therapeutic Art for Positive Wellness.

• Once media has been published, it may not be possible to completely remove it from circulation.

• Therapeutic Art for Positive Wellness cannot guarantee or control the future use of published content by third parties.

You further acknowledge that:

• No payment, royalties, compensation, or other remuneration will be payable in connection with the use of photographs, videos, audio recordings, artwork images, testimonials, or other media.

• Participation in photographs, videos, recordings, or testimonials is voluntary.

• Consent may be withdrawn at any time by providing written notice to Therapeutic Art for Positive Wellness. Withdrawal of consent will apply to future use of media but will not affect materials already published, distributed, or produced before the withdrawal request is received.

• Therapeutic Art for Positive Wellness reserves the right to edit, crop, reproduce, and publish media in a manner considered appropriate for the intended purpose.

Privacy Statement

Therapeutic Art for Positive Wellness collects personal information for the purpose of delivering services, managing enrolments, maintaining records, communicating with participants, and obtaining consent where required. Personal information will be handled in accordance with the Privacy Act 1988 (Cth) and applicable Australian privacy principles.

Personal information will not be disclosed to third parties without consent unless required or authorised by law, or where reasonably necessary to provide services and manage business operations.
